DOT or MC Number Showing Inactive? How to Reactivate It and Fix a BMC-91X Insurance Error (2026)

Few things stop a carrier faster than a broker calling to say your authority shows “Inactive” or “Not Authorized” — especially when you know your insurance is paid. Since FMCSA moved registration into its new Motus system in 2026, this has been happening to a lot of carriers who did nothing wrong.
The key is knowing that “inactive” can mean three different problems, and each one has a different fix. Treat the wrong one and you can lose days.
Step 1: Find out which problem you have
Check your record directly on FMCSA's SAFER Company Snapshot — not a broker's carrier-monitoring software, which can lag behind or misread the status.
| What you see | What it usually means | Go to |
|---|---|---|
| Authority “Not Authorized,” insurance is active and paid | Your insurance filing (BMC-91/91X) didn't sync or doesn't match your record | Fix #1 |
| Authority revoked; you got a notice, or a policy or BOC-3 actually lapsed | Your MC authority was formally revoked | Fix #2 |
| USDOT status “Inactive” | Your USDOT registration was inactivated, usually over a missed MCS-150 biennial update | Fix #3 |
Never apply for a new USDOT or MC number to get around an inactive status. It can trigger FMCSA's fraud checks and flag you as a “chameleon carrier.” Fix the record you have.
Fix #1: Insurance is active, but FMCSA doesn't see it (BMC-91X sync error)
Form BMC-91 or BMC-91X is the electronic proof of liability insurance your insurance company files with FMCSA on your behalf. Most for-hire general freight carriers need at least $750,000 in liability coverage on file. Two things to know:
- You can't file it yourself. Uploading a COI to your Motus dashboard does nothing. Only your insurer can transmit the filing.
- It has to match exactly. The legal name, DBA and physical address on the filing must match your FMCSA record character-for-character. “LLC” vs. “L.L.C.” or an old address is enough for the filing to be rejected without anyone telling you.
How to fix it
- Compare the records. Pull up your Motus/SAFER record and your insurance declarations page side by side. Check legal name, DBA, address and DOT/MC number.
- Call your insurance agent. Ask them to confirm the filing was accepted by FMCSA (not just sent), correct any mismatch, and re-transmit the BMC-91X.
-
Have your insurer contact the Insurance Branch. If the filing still won't post, your insurer can email FMCSA at
FMCSAInsurance@dot.govwith the filing ID, policy number and your DOT number. - Open one support ticket. Submit a single ticket at AskFMCSA with your active COI and the filing date and reference number, and ask for a manual status refresh. Follow up on that ticket rather than opening new ones.
Ask your agent to file your renewal or new policy 10–14 days before the old policy's cancellation date. When a policy is cancelled, the insurer notifies FMCSA in advance — if the replacement filing isn't posted by then, your authority can drop even though you're covered.
Fix #2: Your MC authority was revoked — how to reinstate it
If your authority was actually revoked (not just a sync glitch), you'll need to request reinstatement. According to FMCSA:
- The fee is $80.
- You need active insurance on file meeting the minimum requirements, and a BOC-3 (process agent designation) on file.
- Your USDOT number must be active with current information. If it's inactive, fix that first (see Fix #3).
- Authority is typically active within about a week after FMCSA receives the application and payment.
- Reinstatement isn't available if you were placed out of service for an imminent hazard or have a final unsatisfactory safety rating.
Reinstatement checklist
- Confirm your insurer has re-filed your BMC-91/91X and it shows on SAFER.
- Call your BOC-3 process agent and confirm your filing is active.
- Log into Motus, select Request Authority Reinstatement, and pay the $80 fee.
- Save a screenshot of the confirmation and watch SAFER for the status change.
Fix #3: Your USDOT number is inactive — how to reactivate it
An inactive USDOT number usually means a required MCS-150 biennial update wasn't completed. To reactivate it, submit an updated MCS-150 through Motus with current information.
- Submit once. If your update shows “Pending,” don't submit another — duplicates can freeze the record longer.
- Save your proof. Screenshot the confirmation number and timestamp. Carry a printed copy in the truck while it processes.
- Check FMCSA's Motus notice. FMCSA announced a temporary suspension of USDOT inactivations for carriers who couldn't complete their biennial update because of Motus access issues. Check the page for current status and keep your submission proof either way.

Email them your active COI right away and keep it simple: your authority is caught in an FMCSA Motus system sync error, the load is safe, and you're working with FMCSA to force a refresh. Then call FMCSA at 1-800-832-5660 (8 AM–11 PM ET weekdays, 9 AM–4 PM ET Saturday) and ask for a Tier 2 technical escalation.
Get every template and checklist
The Motus Fix includes a ready-to-send AskFMCSA ticket template for false “Inactive” / BMC-91X sync errors, the exact broker email and phone script, a minute-by-minute checklist for when your authority drops mid-load, and an insurance exact-match worksheet to catch filing mismatches before they ground your trucks.
